Pre-Appointment Paperwork
We believe your first appointment should be all about your comfort, your needs, and your oral health.
We recommend all patients fill out new patient forms and paperwork prior to their first appointment. This will give you and your dentists as much time possible to focus on your treatment during your appointment.

Feel free to give us a call with any questions you have on the form or your first appointment.
What to Expect at Your First Appointment

At Comfort Care Dental, we’re committed to ensuring a comfortable, gentle experience at the dentist’s office. From beginning to end, we guarantee you’ll be in good hands.

Our office is designed to ensure you’re as comfortable as can be. We offer amenities like pillows, drinks, televisions, and blankets to make sure you feel welcome and cozy in our offices.

When you walk through our doors, our friendly staff will be here to help you get the most out of your first appointment. After meeting your dentist, we’ll begin your comprehensive oral exam, which includes…
-
A full set of x-rays and a review of your physical and oral health history
-
Through examination of your teeth and gums
-
Full oral cancer screening
-
Plenty of time to have your questions and concerns answered by your dentist

Once the exam is complete, together with your dentist we’ll formulate a dental treatment plan you’re comfortable with.
We’ll leave no stone unturned and make sure we’re putting our best foot forward to get your oral health on the right track. There will also be an opportunity for you to review insurance and financing information with our front office staff if you haven’t already.

We can’t wait for you to see what comfortable dentistry means to us.

Modern sedation options allow you to receive the dental treatments you need while still allowing you to be comfortable. Our sedation options are gentle, comfortable, and calming. You won’t believe how sedation dentistry can change your relationship with your oral health!
